  • Ensures that clients stay connected to their behavioral health provider by facilitating engagement during and after their hospitalization, with the ultimate goal of decreasing hospital recidivism. Following a hospitalization, this team with engage with the client, notify their team, and get direct supports in place quickly to promote an earlier discharge and prevent re-admission.

  • Provides crisis stabilization services for children/caregivers/families to reduce the need for re-hospitalization.

  • Provides support and resources to clients and families that are experiencing acute behavioral health crisis, reduces placement disruption and ensures stability while involved with the Department of Child Safety and fosters independence of children and families that receive JFCS services.

  • Provides advocacy, resources, and support to the Children’s Rehabilitative Services population consisting of children and young adults with medical and behavioral health issues. Coaching Club

  • Helps to identify the function of the child’s behavior and provides intensive behavior coaching that works with the child, parent/caregiver, and the environment to create change in the child, their support network, as well as their environment.

  • Services provided to adult that have a SMI determination: Medication management, case management, treatment/discharge planning, vocational/employment support, therapy referrals, housing support, peer support, meaningful activities.

  • Helps to prepare youth and young adults for their movement into adult roles through an individualized process that incorporates a wide range of activities to include life skills, budgeting, resume writing, job development, social skills, and counseling.

Child & Family Solutions Programs

  • Provides resiliency-based student assistance and prevention services designed to promote the social, emotional, and mental health of students in elementary, middle, and high school settings.

  • Engaging and empowering families to build support systems, enhance community resources, and address individual needs in order to create a safe, nurturing, and stable environment for their children.

  • Helping to empower young adults by offering individualized, high-quality programming addressing independent living skills and employment training to support youth transition successfully to adulthood.

  • SWW offers community-based assistance to individuals impacted by domestic violence residing outside of a shelter setting.

Jewish Community Services and Older Adult Programs

  • Providing assistance for Jewish families facing financial crisis, those needing resources during holiday seasons, older adults who may benefit from additional help, and those needing behavioral health services.

  • Provides older adults with comprehensive in-home support such as counseling, case management, advocacy, and referral to appropriate resources.

  • Responsible for the management and maintenance of the electronic medical records system. Develop and monitors data quality metrics to ensure business data and reporting needs are met.

  • Responsible for billing claims to each payer and resolving any barriers to payment.

  • Provides reasonable assurances that JFCS is complying with local, state, and deferral regulations as well as any other governing or accrediting body.

  • Provides customer service to clients through the completion of intake referrals for initial services.

  • Verifies and notifies each client of their benefits and financial responsibility prior to treatment and assists with obtaining prior authorization as needed.

  • Ensures the organization’s financial assets are protected through the implementation of sound financial policies and procedures, and to provide stakeholders with meaningful financial data that is in compliance with regulatory and contractual requirements.

  • Manages the needs of company employee. From organizational development through employment law interpretation, HR helps in maximizing the efficiency of the organization by focusing on human capital and organizational success.

  • Collaborates with staff to design, develop, test, and implement effective technology-based solutions in support of the business strategy, goals, and operation.

  • Connects individuals, corporations, foundations, and volunteers with JFCS to provide financial and other support to agency programs. Oversees the marketing and public relations for the agency to communicate and promote JFCS with potential clients and key stakeholders.

  • Proactively and systematically works to improve the quality of services provided at JFCS.

  • Responsible for defining strategies, tasks, and methods that will be used to meet all training requirements.
